MI Choice Medicaid Waiver

Our Person-Centered Planning approach honors an individual's preferences, choices and abilities when planning care.  Consultants support, educate and encourage the care recipient in decision making, but do not make the decisions on their behalf. 

Once the planning process is complete, care recipients also have the choice of whether they would like an Area Agency on Aging 1-B Care Manager to coordinate and manage care services on their behalf or they would like to manage these services themselves.

 

The MI Choice Home and Community Based Medicaid Waiver is a program where Medicaid pays for services in an individual's residence (home, caregiver's home, assisted living facility). To qualify an individual must be 18 years of age or older, require a nursing home level of care and have a montly income of $2,022 or less and assets no greater than $2,000.

For qualified individuals the MI Choice program will help cover services such as personal care, homemaking, adult day services, in-home respite programs for the caregiver, meals on wheels and personal emergency response systems.
